Infection is the sequel's signature threat: it's hidden at first, it escalates on a timer, and an infected teammate who hides it can wipe a squad. This is the complete system — causes, symptom order, cures, and the grim last resort.
The three ways you get infected
- Grabs: zombies grab and start a button-mash minigame. Fail it — or get grabbed while under 15% stamina — and the bite infects you. Lower stamina makes the minigame harder, which is why exhausted melee players get infected most.
- Low-health hits: at 50% health or below, any zombie hit can infect. The lower your health, the higher the odds — bandage up before pushing.
- Shambler gas: a killed Shambler vents gas for ~20 seconds; standing in it is a near-guaranteed infection. Zombies coated by the cloud also infect on hit at a much higher rate.
Symptom stages — how long you have
After a short symptom-free incubation, the infection announces itself in a fixed order:
- Stage 1: "migraine aura" — faint white god-rays at the edges of your vision, then blur.
- Stage 2: audio takes on an echo; your Responder starts coughing — audibly, to the whole squad.
- Stage 3: whispers ("We'll see you soon…"), violent coughing, and finally The Veins — dark lines crawling into your vision. If you don't medicate now, you die on the spot.
Cures: Phalanx Pills vs Gene Therapy
- Phalanx Pills (semi-common): reset infection progress. Each bottle holds 3 uses — the standard play is to wait until The Veins before popping one, so a bottle covers three full infections.
- Experimental Gene Therapy (rare): a single-use syringe gun. Use it once to draw blood, wait for the antibody progress bar, then use again to inject — curing you and granting immunity for the rest of the session. Give it to whoever is infected rather than using it preemptively; community testing also reports a small chance of ~10 seconds of blindness after injecting.
- Extraction: infected Responders who extract are cured automatically.
The self-sacrifice option
Infected with no pills and no therapy? With a loaded firearm you can press K to go out on your own terms — a returning feature from the original game. The payoff: your body doesn't turn and chase your squad; it drops your loot cleanly. Use it only at the verge of turning.
